Some members of Dental Health Services, a California Company, and Dental Health Services, Inc., a Washington Company may have had personal information disclosed in a privacy incident.

What Happened?
On February 7, 2024, the Company became aware of the unauthorized disclosure of information to some of its group insurance customers. The Company learned that it had emailed incorrect invoices containing personal information to some employer groups. The emailed invoices were encrypted with password protection but each group email recipient was sent the encryption password in a separate email to view the invoice.

What Information Was Involved?
The incorrectly disclosed benefit invoices contained each individual's name, date of birth, member identification number, eligibility date, plan type, and premium amount due.

What Are We Doing?
Upon discovery, we took prompt action to investigate the matter. All the email recipients have confirmed destruction of the incorrect invoice. The Company has since modified invoicing software code to create additional safeguards preventing incorrect invoicing of the type revealed by this security incident.

Please check your explanation of dental benefits statements, bills and accounts to be sure they look correct.
